Senior Care Physician
We're looking for a Senior Care Physician to join our team and play a key role in providing direct patient care primarily in the ambulatory healthcare center setting. The responsibilities include geriatric assessment, medical history, physical examination, diagnosis and treatment, development of the plan of care, health education, specialty referrals, case management referrals, follow-up and clear documentation according to company standards for quality, service, productivity, and teamwork.
- Function independently as a primary care practitioner as part of a patient care team.
- Assess acute and non-acute clinical problems and develop patient management plans.
- Participate in patient care rounds and conferences and collaborate with the multidisciplinary team to ensure successful patient management strategies.
- Utilize advanced communication skills to problem solve complex situations and improve processes and service to patients.
- Advocate for improved access, quality, and cost-effective healthcare.
Requirements:
- Current licensure in the state of practice is required.
- Certification in Basic Cardiac Life Support required.
- Board certification in Internal Medicine or Family Medicine is preferred.
- Minimum of one (1) year clinical experience in geriatric, adult or family practice setting preferred.
