Position Overview: The Physical Therapist - Rehabilitation Program Coordinator is responsible for assessing, planning, organizing, and participating in rehabilitative programs that improve patient mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. This role also involves providing clinical supervision of rehab aides.

Job Responsibilities:

  • Design and implement individually tailored physical treatment programs to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients
  • Complete and document patient evaluations and tests per established standards and regulatory requirements
  • Develop and update patient's Plan of Care (POC) based on assessment data and diagnosis
  • Assess and provide/progress treatment for patients according to the plan of care
  • Coordinate patient care effectively with other care provider and support team members
  • Provide clinical supervision to rehab aides

Qualifications:

  • Graduate of an accredited physical therapy program
  • PT-Physical Therapist license required
  • BLS-Basic Life Support Healthcare Provider certification required
  • 2 years of recent relevant experience preferred
  • Strong critical thinking, problem-solving, and communication skills

Work Schedule: Short Hour, 8 hours per shift, variable days of the week with rotating weekends

Compensation: Pay range is $60.10 to $75.12 per hour

The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate's experience, education, skills, licensure and certifications, department equity, training and organizational needs.
