As an RN Home Health Field Supervisor at VNS Health, you will provide case management support, including clinical intervention, care plan development, and follow-up support. Mentor and lead Home Health Aides to ensure top-quality patient care. This role offers a three-day flex schedule, generous paid time off, health insurance, retirement savings, and professional growth opportunities.
Job Responsibilities:
- Conduct initial clinical and supervisory field visits to patients' homes
- Document patient visits in the EMR
- Orient and supervise Home Health Aides in the field
Requirements:
- Current RN license in New York State
- Minimum of two years' experience in home care
- Valid driver's license may be required
