This position requires a valid Pennsylvania Registered Nurse license, a valid Pennsylvania driver's license, and reliable internet access. As Homeland HomeHealth's RN you would provide provide skilled professional nursing care to home health patients, as prescribed by the physician and in compliance with the Home Health Care Medicare Conditions of Participation, Pennsylvania Home Health Care licensure laws, and agency policies and procedures. You will provide the initial and comprehensive nursing assessment of patient needs and collaborate to develop the home health plan of care, as well as reevaluate and revise the plan of care relative to the patient's nursing needs to provide appropriate, comprehensive and responsive goals and interventions. You will work closely with other members of the health care delivery team to ensure all patient and family care needs are being met and provide supervision to Licensed Practical Nurses (LPN) and Home Health Aides. Requirements include: Education and Experience Requirements: Graduate of an accredited registered nursing program. Bachelor's degree in Nursing preferred. Has minimum of one (1) year general nursing preferred with current home health, medical, surgical or critical care experience. Ability to work within an interdisciplinary setting. Ability to work independently. Licensure, Registration, and Certification Requirements: Registered Nurse licensed in the Commonwealth of Pennsylvania. Valid driver's license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy. Satisfactory references from employers and/or professional peers. Satisfactory criminal background check. Maintains current CPR certification. Access to reliable internet access.
