Social Worker-Behavioral Health - Quannacut Detox

SCHEDULE: 3 SHIFTS PER WEEK AND 1 4TH SHIFT IN A 4 WEEK SCHEDULE.

SHIFTS HOURS CAN VARY FROM 8A-8P, 11A-11P.

SCHEDULE- 8A-4P**, VARIED DAYS 5 DAYS PER WEEK REQUIRED.

Position Description:  

 At Stony Brook Eastern Long Island Hospital, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings.  Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; substance use; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention.

SPECIAL SKILLS / EDUCATIONAL REQUIREMENTS:

Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

  • Provides group, individual, and conjoint therapy.
  • Charting and documentation
  • Assist patient financial services, including Medicaid and other entitlement programs.
  • Serve as a liaison between groups and individual therapy.
  • Supervision of professional social work students when appropriate.
  • Actively involved in performance improvement activities including program development, data collection, and analysis.
  • Demonstrates knowledge of DSM 5 diagnoses, housing, and aftercare services within Suffolk County.
  • Can identify and address psychosocial assets and stressors that may impact the patient's process.
  • Adheres to NASW, CASAC and hospital Code of Ethics.
  • Can provide timely and effective documentation on the medical record.
  • Works collaboratively with the treatment teams throughout the hospital.
  • All other duties as assigned.

Qualifications:   MSW from an accredited Social Work Education School.   NYS LMSW or LCSW.  Three year’s post Master’s Social Work experience in Behavioral Health.   Demonstrates skills in crisis intervention.  Demonstrates understanding of ED discharge processes.  Demonstrates excellent written and oral communication skills.  Ability to work and coordinate with established Medical and Behavioral Health Treatment Teams.  Demonstrates understanding of discharge planning. Demonstrates good assessment and psycho-social skills.  Working knowledge of Long Island community resources.  Demonstrates patient centered, patient advocacy and service excellence.  Computer literate.   Demonstrates understanding of community systems. Demonstrates teamwork.  Demonstrates understanding of data collection and statistics.  Demonstrates ability to manage complex caseloads. 

Preferred Qualifications:  Experience in Emergency Department Social Work.  Experience in Hospital, In-Patient settings.  Discharge Planning experience.  Diagnosis specific Social Work.  Bi-lingual.

  • Successfully complete a 4-panel drug screen*
  • Meet Regulatory Requirements for pre-employment screenings.
  • Provide a copy of any required New York State license(s)/certificate(s).
