A Comprehensive post-acute continuum health systems company is interested in hiring a Admissions Coordinator to their team onsite in Woods Cross, UT
Description:
-
Coordinates admissions for homecare, hospice, and skilled nursing facilities
-
Obtains pertinent pre-admission documentation from referral sources to determine if agency can care for the patient adequately
-
Review clinical documentation to determine level of care needed
-
Verifies physician orders
-
Develops and maintains relationships internally and with community partners
-
Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
Skills & Qualifications:
• CNA/MA or Knowledge of Skilled Nursing, Homecare, and Hospice criteria , but not required
• Minimum 1 year experience in healthcare
Pay and Benefits
The pay range for this position is $19.00 - $19.00/hr.
