Position Summary

The Quality Assurance Nurse will work alongside the Director of Accreditation and Quality to oversee a comprehensive quality assurance program for all health center locations. Ensure that quality assurance is functioning effectively and identify and resolve any clinical issues impacting patient health and safety. The QA Nurse will also function as the employee health nurse and risk management liaison with overseeing all pre-employment testing as well as all incident reports. This position will monitor and track incident trends and then educate staff on how to avoid those incidents in the future.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:
  • Works with the Director of Accreditation and Quality to implement a robust quality program to meet all Indiana Medicaid MCE reporting requirements, Gaps in Care, EPSDT, HEDIS, and other high priority clinical quality assurance projects
  • Attend and participate in regulatory HRSA site surveys and Joint Commission Surveys for all health center locations. Assist in writing and/or approving plans of correction in response to deficiency statements and ensure appropriate follow-up and correction actions are taken.
  • Help disseminate information and educate all Family Health Center staff (individually or in groups) in regard to quality initiatives, best practices, survey preparedness, risk management, organization policy and applicable state/federal regulations
  • Have broad understanding of HRSA UDS quality measures as well as TJC requirements
  • Attend the bi-monthly Board Quality meetings as well as the prescriber's meetings
  • Champions the internal event reporting system to improve uptake and use among all staff while creating a "just culture" to ensure staff feel safe reporting adverse and near-miss events
  • Follow-up on any patient complaints logged within the internal event reporting system
  • Identifies areas of potential risk and develops systems within the organization to minimize risk of adverse events or negative quality outcomes
  • Conducts Root Cause Analyses for clinical adverse events and near miss events; develops action plans to reduce risk of recurrence, follows the progress of these action plans
  • Demonstrated proficiency in the application of project management and performance improvement tools.
  • Annually Reviews & Revises the Performance Improvement Plan in cooperation with the BOD, Executive Leaders, Operations Managers, Director of Clinical Operations & Director of Quality and Accreditation.
  • Analyze patient/client surveys and quality data to identify opportunities for improvement.
  • Assist in departmental review of clinical aspects of care and preparing PI reports for submission to the appropriate teams/committees.
  • Consult with teams on performance excellence methodologies and provide education to support teams in improvement work.
  • Incorporates understanding of the Family Health Center's mission, goals, and objectives into individual team activities.
  • Acts independently while seeking guidance and direction as necessary for the successful completion of job duties.
  • Will monitor employee work-related illness and injury trends and assesses, develops and implements strategies to prevent work-acquired injury or illness and expedite return to work
Minimum Qualifications (Knowledge, Skills, and Abilities)
  • Must have a valid IN Registered Nurse license
  • Must have an associate's degree in nursing and pursing a BSN with completion within 3 years of hire
  • Five years of nursing experience required with at least 3 years of experience in quality assurance, quality improvement, and/or regulatory readiness activities.
  • The QA Nurse will need clinical nursing experience. The QA Nurse must be generally familiar with regulatory readiness, able to develop an in depth understanding of HRSA Site Surveys and Joint Commission standards, and eager to remain up to date on changes to these requirements. The QA Nurse will need to possess excellent organizational skills, a proactive approach, and an ability to collaborate across roles and disciplines. The QA Nurse should be able to interact effectively with all levels of staff, discuss negative events with others in a supportive and non-punitive way, and inspire others around him or her toward a goal of continual improvement.
  • The QA Nurse will participate in health center committees and department management meetings as needed. The QA Nurse should have strong analytical skills, should be independent and self-motivated yet eager to work with others, and should be facile with interpreting numerical outcome data and presenting it visually (e.g. Excel/PowerPoint)
