Overview

Agrace is seeking a quality-focused Nurse Practitioner (APNP) to join our collaborative team in providing the exceptional care and positive patient/family experience for which Agrace is well known. This is an excellent opportunity for the practitioner desiring significant and meaningful time with patients and families in their homes, primarily on a weekday schedule.

Responsibilities

The Supportive Care Nurse Practitioner (APNP) is a member of the medical services team and the palliative care consultative team. This position collaborates with patient’s primary care providers in the assessment, planning, implementation and evaluation of community-based palliative care interventions. This position is responsible for interdisciplinary collaborative practice, ongoing quality assessment and performance improvement, performance management, financial management, and ensuring exceptional care delivery consistent with hospice/palliative care philosophy and best practice. This position requires in-home visits with our patients along with telemedicine visits as needed.

Qualifications
  • Current WI RN license, Advanced Practice Nurse Prescriber (APNP) licensure, and national AANP or ANCC NP certification.
  • Master’s Degree in Nursing
  • Hospice or palliative care experience and/or certification preferred or expectation that certification will be obtained within 2 years of hire.
  • Experience with and knowledge of health care policies and coding/reimbursement practices, state and federal health care regulations, standards and procedures and home health and hospice certification and accreditation preferred
  • Epic knowledge desired along with an excellent comfort level with the use of technology.
  • CPR certification preferred (required within 90 days of hire)
  • Valid driver's license and access to an automobile in reliable operating condition. This position requires daily travel.
