Overview
Responsible for coordinating admissions of hospice patients by making skilled assessments of patient's initial physical, psychosocial, and spiritual needs.
Responsibilities
- Educates patients and caregivers regarding the hospice program, philosophy of care, obtains informed consent, election of benefit, and reviews patient rights and responsibilities and other required federal/state documents.
- Performs initial assessment of physical, environmental, spiritual, bereavement and psychosocial factors to determine hospice patient and family needs upon admission and establishes the Plan of Care.
- Coordinates the establishment of the initial plan of care with the attending physician, the Medical Director, and the Interdisciplinary Team.
- Obtains verbal certification of terminal illness from attending physician (if any) and Hospice Medical Director.
- Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of Physician.
- Coordinates the total Plan of Care and maintains continuity of patient care by communicating and coordinating with appropriate staff. Coordinates for the provision of DME, supplies and medications identified in the plan of care. Revises the plan of care as needed. Completes updated Plan of Care according to state and federal regulations and communicates changes to attending Physician, hospice staff, and contractors providing patient care.
- Functions as a liaison between disciplines and facilities during the admission process.
- Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
- Communicates significant findings, problems, changes in condition or environment to the Clinical Manager/Physician and other members of the hospice care team. Communicates with the Physician regarding the patient's needs and reports changes in the patient's condition; obtains physician orders as required.
- Provides appropriate medical intervention per physician orders to palliate symptoms.
- Participates in care center QAPI program as requested.
- Participates in care center's performance improvement plan and Interdisciplinary team meetings as requested.
- Performs on-call responsibilities and on-call services to patients/families as assigned.
- Complies with policies, procedures and regulatory mandates including but not limited to abiding to the terms of the Amedisys Compliance Program.
- Performs other duties as assigned.
Qualifications
Required
- Current, unencumbered license to practice as a Registered Nurse specific to the state(s) the employee is assigned to work/cover for the company. May possess either a single state license in each non-compact state and/or a compact license in their state of residence that is accepted in all Nurse License Compact (NLC) participating states.
- Nurse License Compact (NLC) documents the following: The mutual recognition model of nurse licensure allows a nurse to have one license (in his or her state of residency) and to practice in other states (both physical and electronic), subject to each state's practice law and regulation. Nurses must legally reside in an NLC state to be eligible to have a multi-state license.
- One (1+) year of experience as a Registered Nurse.
- Current CPR certification.
- Knowledge of physical, psychosocial, and spiritual needs of terminally ill patients and their caregivers.
- Prior Hospice experience.
- Computer literate, proficient in windows navigation, emails, internet and documenting in an EMR. Able to self-manage online HR services and online training programs.
