A Community Health Worker (CHW) serves as a crucial link between Sanitas medical centers and the surrounding community. The CHW is responsible for connecting with local resources, mapping the community, and linking leaders and services to our users.

Community Health Worker Roles and Responsibilities:
  • Identify community resources at the local level, connecting with various organizations, leaders, and local clubs (e.g., park activities, walking groups, knitting clubs).
  • Collaborate with local organizations to offer and organize community activities and develop strategies together.
  • Create and promote group activities within and outside our centers to better reach the community, addressing issues like loneliness (e.g., Talking Café, physical activity, themed day activities for the elderly).
  • Facilitate self-sustained groups by identifying and promoting potential group leaders within our patients and the broader community.
  • Identify key community connectors, train them, organize regular meetings to understand community needs, and track community referrals.
  • Support patients with the highest needs by connecting them to resources when extra help is needed.
  • Collaborate with the medical center team and participate in weekly meetings to ensure a community-focused approach.
  • Report to the Community Coordinator in Tennessee.
  • This position has no supervisory responsibilities.
Skills & Qualifications:
  • High school diploma or equivalent and CHW certification in Tennessee required.
  • 1-year experience in community development and/or community outreach required.
  • Knowledge of community relations management, including understanding local community needs and the ability to conduct interviews.
  • Public speaking skills required.
  • Administrative skills required.
  • General knowledge of Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
  • Relevant vocational coursework preferred.

Physical Demands: The physical demands described here are representative of those that must be met by an employee to perform the essential functions of this job. The employee is regularly required to stand, walk, use hands to operate tools and equipment, and must be able to exert up to 10 pounds of force regularly, 30 pounds frequently, and 50 pounds occasionally. The employee will frequently reach with hands and arms, bend, balance, kneel, crouch, crawl, push, and pull. Specific vision abilities required include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus. The employee is subject to environmental conditions, noise levels that may require shouting to be heard, and protection from weather conditions but not necessarily temperature changes.
