About the Opportunity:

  • Schedule: Monday through Friday
  • Hours: Standard business (35 per week
  • Program/Department: Aging and Nutrition Services
  • Patient Population: Adults / Geriatrics (Ages 55+)
Responsibilities:
The Social Worker (LMSW) will:
  • Identify depression, anxiety, and substance use disorders using standardized tools
  • Collaborate with support staff to refer clients promptly to mental health professionals
  • Coordinate monthly Group Supportive Counseling and Mental Health Education sessions
  • Document all interactions and referrals accurately in STARS and the database
  • Provide Case Management and develop care plans to support aging in place
  • Prepare documentation as per agency standards
  • Perform other duties, as needed

Qualifications:
  • 2+ years of Social Work experience
  • Bachelor's Degree in Social Work
  • Active NYS Licensed Master Social Worker (LMSW) license
  • Bilingual (English and Cantonese, Mandarin, or Spanish)
  • Experience with geriatric populations, mental illness, substance use, assessments, and care coordination.
Desired Skills:
  • Master's Degree in Social Work
