MDS/Careplan Coordinator

Our MDS/Care Plan Coordinators are responsible for coordinating the development of an individualized comprehensive Care Plan that includes measurable objectives and timetables to meet each resident's physical, emotional, and social needs.

Schedule: Full-Time Monday - Friday

Starting Wage: $45hr

Qualifications:

• Registered Nurse with current unencumbered state licensure
• Prefer experience with MDS 3.0
• Prefer experience with PDPM
• Ability to read, write, speak and understand the English language

• Schedules OBRA and PPS assessments for each resident based on RAI scheduling guidelines
• Conducts accurate and standardized assessments of each resident in the facility.
• Codes designated sections of the MDS document for each resident.
• Completes CAAs on all comprehensive assessments for each resident in the facility.
• Reviews the MDS data for accuracy.
• Coordinates the completion of the MDS and care planning process by all disciplines.
• Completes all OBRA and PPS assessments.
• Submit MDS assessments to iQIES and verifies that assessments have been transmitted and approved by the State in a timely manner.
• Corrects any rejected records and prepares them for re-submission.
• Review admission records and enter medical diagnosis based on ICD-10 guidelines
• Initiate baseline care plan and ensure timely completion
• Completes care plan process to accurately reflect resident's status
• Leads the Interdisciplinary Care Plan Team Meetings
