About the Role:

We are looking for a compassionate and dedicated Registered Nurse to join our team and provide personalized care to hospice patients and their families. In this role, you will serve as the primary liaison between physicians, patients, and the hospice team, offering direct patient care services and supporting the patient's family with empathy and trust.

Key Responsibilities:

  • Report to the Administrator, Executive Director, or Patient Care Manager Senior
  • Act as the primary liaison between physician, patient/family, and hospice team
  • Provide direct patient care services and supportive care to the patient's family
  • Empower patient's caregivers through education
  • Manage and collaborate with an interdisciplinary group to provide comfort and dignity
  • Evaluate patient and family needs and assist in the preparation and execution of the care plan
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

Qualifications:

  • Compassionate and empathetic with a strong commitment to hospice care
  • RN license in the state of employment
  • 1+ years of nursing experience in hospice, home health, ICU, geriatrics, or oncology preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
