What you’ll do:

As anROE Pharmacist PRN, you will beresponsible for prospectively reviewing and ensuring the safe and effective use of medications, following hospital policy and making recommendations and interventions as necessary, for LifePoint Hospitals that do not have 24-hour pharmacy coverage.

Responsibilities:

Verify practitioner orders, ensure doses are within dosing limits, and screen orders for drug interactions in a timely manner.

Screen all orders for drug interactions, allergies, order duplications, renal, and liver function prior to order verification or entry.

Responsible for using clinical knowledge to make decisions and recommendations on the appropriateness of pharmaceutical therapies that are ordered after the respective pharmacies have closed for the day.

Review and interpret all physician orders received, using the patient profile and assess orders for age-specific appropriateness from neonatal through geriatric.

Assist in meeting overall operational order entry goals and clinical program goals to include, but not limited to, therapeutic interchanges set forth by the Company.

Follow facility procedures when non-formulary drugs, target drugs, restricted drugs and investigational drugs are ordered.

Perform clinical patient management of antibiotics, anti-coagulants, and all other meds requiring adjustments, including renal adjustment of medications, lab ordering, kinetic adjustment of therapy, lab based adjustment of therapy, ordering patient meds based on clinical status and response to therapy.

Document clinical interventions, follow-up when indicated and enter medication orders that are faxed/scanned using several different Electronic Health Records (EHR), such as Meditech, Medhost , Meccomm, and HMS systems, efficiently.

Clarify, investigate, resolve, intervene, and consult with physicians and nursing staff when questions regarding medications, allergies, and therapy guidelines arise.

Facilitate dissemination of drug information in a timely manner to achieve high quality, cost effective, positive therapeutic outcomes when consulted.

Prepare written or oral report(s) at shift change to communicate incomplete assignments and potential patient safety concerns, monitor order(s) to ensure continuity of medication therapy and articulate issues for continued standardization amongst facilities and quality improvement.

Maintain and ensure orders adhere to state and federal compliance, as well as compliance with The Joint Commission (TJC) and hospital standards.

Interchange medications to match drug formularies and convert IV (intravenous administration) to PO (oral administration) therapy when appropriate per protocol.

Regular and reliable attendance.

Perform other duties as assigned.

What you’ll need:

*Education:*Graduate of an Accredited Pharmacy School

*Experience:*Minimum of 3 years’ as inpatient/acute care hospital pharmacist. Relative system experience preferred.

*Licenses:*Pharmacy license in the state of Kentucky
