Qualifications Minimum Qualifications Current Licensure in Social Work or in a clinical or related discipline (RN preferred), per state guidelines for case management and/or discharge planning, required. Previous Experience Previous experience in Case Management and Discharge Planning preferred. CCM Certification Preferred.
