  • PART TWO: FUNCTIONAL RESPONSIBILITY
    • Position Objective
      • Provides documentation, charting, MDS completion and Care planning input for all patients. Is responsible for the nutritional management of acute care patients . He/she works with the Occupational and Community Health Departments offering nutrition programs as time permits. Helps supervise APD kitchen staff to provide quality food and service to patients, residents and staff.
    • Performance Expectation
      • Assists staff in menu correction for therapeutic diets, review of menus for completeness, and accommodation to patient food preferences.
      • Provides menu revision and approval for all three communities (Harvest Hill, APD and ECF) with therapeutic options available at each meal. Completes spreadsheets for therapeutic diets. Provides education and teaching to Director and food preparation staff. Helps Food Service director with conducting inservices for sanitation and infection control.
      • Supervises the patient tray line to ensure proper serving temperature, taste, texture and accuracy in trays, compliance to diets and overall appealing appearance. Enforces all infection control policies. Monitors temperatures on serving line and on trays. Assures that all trays are garnished.
      • Enters complete and accurate information in the patients' medical records using the Soap method of charting.
      • Shows competency in providing care appropriate to the age of our general patient population (adult geriatric).
      • Develops plans of action for patients/residents at nutritional risk and maintains communication with Director of Dining Services and kitchen lead persons.
      • Monitors patient menus for adequacy and appropriate diet modification.
      • Conducts patient education for modified diets or as needed and documents patients' understanding of information materials.
      • Communicates with the dietary staff regularly concerning patient diets, problems, requests, etc. either verbally or through the communication book. Assists staff with updates to the Cardex, making appropriate diet changes as necessary.
      • Supports the Food Service Director in tasks related to clinical management and menus.
      • Recommends tube feeding products and educational materials.
      • Keeps Department Manager up-to-date regarding clinical activities.
      • Represents the hospital in promoting nutrition and wellness programs.
      • May require flexibility in hours.
      • Participates in Community and Occupational Health Programs as time permits.
      • Maintains updated patient records.
      • Keeps up to date on current nutrition theories and practice.
      • Develops and collects up-to-date patient education materials.
      • Communicates with physicians and nurse managers regarding the nutritional care of their patients as appropriate.
      • Develops nutritional quality assurance programs.
