We are seeking a Physical Therapist (PT) for a 13-week contract at a premier Skilled Nursing Facility (SNF) in Carmichael, CA. Key Responsibilities:
  • Perform comprehensive assessments of patients' physical conditions and capabilities.
  • Design and implement personalized treatment plans to improve mobility, strength, and overall wellness.
  • Collaborate with a multidisciplinary team, including physicians, nurses, occupational therapists, and speech therapists, to provide well-rounded care.
  • Educate patients and families on exercises and techniques to support recovery and independence.
  • Accurately document patient progress, treatment interventions, and outcomes.
  • Participate in team meetings and engage in continuous education to stay current with physical therapy advancements.
Qualifications:
  • Doctorate or Master's degree in Physical Therapy from an accredited institution.
  • Valid California Physical Therapy license.
  • Experience in a skilled nursing facility or similar healthcare setting preferred.
  • Excellent communication and interpersonal skills.
  • Compassionate and patient-focused approach to care.
Schedule:
  • Day shifts available, with flexibility based on patient needs and your availability.
  • 13-week contract with the possibility of extension based on performance and facility needs.
