Reports to: Program Director Trans Health Team

Department: Trans Health Access Team

General Responsibilities:

The Behavioral Health Clinician is a member of the Trans Health Access Team; a multi-disciplinary team comprised of medical providers, behavioral health providers and support staff who provide care for our Trans patient population. The Trans Health Access Team supports patients of Thundermist Health Center including those who identify as transgender, genderqueer & gender non-conforming through an Informed Consent based treatment model in an intergraded primary care setting. The Behavioral Health Clinician for the Trans Health Access Team is an integral member of this team, they have advanced clinical and cultural competency expertise with the trans community and knowledge of trans community resources and needs. They may, on occasion, also help facilitate internal/external consultation & trainings, assist with other Trans Health Program activities and represent the program at community events.

Qualifications:

Required Qualifications:

  • Master's degree in Social Work and clinical experience working with the transgender community.
  • Possess a RI professional license (LCSW) or are scheduled to sit for the LCSW exam.
  • Provide own reliable transportation with proof of RI minimum requirements of auto insurance (including a rider to utilize your automobile for work purposes).
  • Previous experience using an electronic health record preferred.
  • Experience working on a multi-disciplinary team in a medical environment.
  • Advanced clinical and cultural competency knowledge of the transgender community.
  • Experience/comfort with leading groups and facilitating trainings.

  • Bilingual/multicultural candidates (Spanish/English).

Significant Job Functions:

Clinical & Direct Service Responsibilities
  • Key member of the Trans Health Access team collaborates with physicians, other care team members (Medical Assistants, Nurses, etc.), hospitals, partner agencies, and others involved with a patient's network of care.
  • Assess clinical emergencies, including risk of suicide and danger to others.
  • Delivers individual, group, couples and or family interventions.
  • Applies knowledge of Psychiatric Diagnosis (DSM 5 & ICD-10) to perform and record mental status examination findings and billing codes with assigned patient caseload.
  • Apply knowledge of methodologies of psychotherapy to provide individual, group, and family behavioral health counseling.
  • Provide referrals to appropriate resources and follows-up as necessary.
  • Obtain progress notes from outside providers involved in patient care.
  • Engage and empower patients to understand their health issues and work closely with patients to take proactive measures that will result in positive outcomes.
  • Facilitate internal & external Trans cultural and clinical competency trainings on occation.
  • Duties can change as program requirements evolve.
Documentation & General Administrative Duties
  • Perform timely and accurate record keeping in accordance with all Health Center requirements.
  • Complete daily activity logs, behavioral health screening & progress notes, encounter forms, reports, evaluations, and insurance documentation as required by pay sources.
  • Complete clear, specific, and behaviorally measurable treatment plans with assigned patients.
  • Maintain cooperative relationships with Health Center employees, patients, community agencies and the public.
  • Work in cooperation with co-workers and supervisory staff.
  • Collaborate with team members resolving problems in a constructive manner.
  • Maintain a willingness to listen to and respond to constructive feedback from supervisory personnel.
  • Serve on Health Center committees and workgroups as requested.
  • Perform other duties as assigned for which you are qualified
Physical Effort/ Environment:
  • This is a hybrid role and may be done remotely or at a Thundermist location.
  • This position is primarily office based with some community-based activities.
  • Moderate physical activity is required: sitting, standing, walking, and lifting.
  • Travel to off-site meetings and other program sites as needed.
  • General office environment: manual dexterity needed for keyboard work, writing and phone calls.
  • Demonstrated experience working with Electronic Health Records, computers and other technology and Microsoft Office applications.
  • Use of technology for home monitoring, telemedicine, and other devices, as needed.
Work Schedule Demands:
  • Full-time (40 hours per week). Ability to work evenings and weekends as required.
Communication Skills:
  • Strong communication skills, both oral and written.
  • Strong working knowledge of health literacy skills (e.g. Teach Back)
  • Ability to relate to patients and build trusting relationships.
  • Ability to communicate effectively with co-workers, patients, family/care givers, and partner agencies.
