Summary:


The MetroHealth Community Health Center is looking for an APP to support our growing programs. We are seeking a provider who can provide care to patients across the lifespan with primary locations being in Collinwood and Euclid. Community Outreach within the FQHC-LAL continues to grow, and your clinical location may change daily (schedule known in advance) at a community partner clinic (ex: food bank) or a mobile unit (ex: homeless shelter). This APP will serve patients across the lifespan (pediatric through geriatric), offering comprehensive primary care, telehealth services, chronic disease management, and care coordination with our integrated team. Join a committed team that meets people where they live, work, learn, and play.

  • Family Medicine certification and experience required.
  • 1 year of APRN experience required.
  • 2 years of APRN experience preferred.
