SHARE Sonoma County (SHARE), a 501c3 nonprofit, offers housing through several housing programs: its traditional Home Sharing program and Community House program. This case management position is offered to support our Community House program which utilizes empty houses to create community through shared housing. This housing program supports clients who are currently experiencing homelessness and serves those mostly individuals who are 50 years and older and cannot afford housing in this county due to extremely low-income status. Job Description: Provide case management, must speak proficiently in both English and Spanish languages, through assessment and ongoing supportive services designed to assist each client to stabilize through housing, utilizing our Stepping Stones to identify and provide additional helpful resources to encourage improved health, well-being, and financial status. Bilingual Case Manager Job Responsibilities: 40-Hours Full-Time Case Management position: Salary based upon experience Responsible for supporting the needs of each client participating in the SHARE Community House program throughout Sonoma County Client Supportive Services: Treat each client in a nonbiased and nonjudgmental manner at all times Establish case management profile/assessments: Identify vulnerabilities of each client History of addiction and addiction type Behavioral Health Diagnosis Determine case plan / Stepping Stones application for each client Identify applicable resources to help improve each clients well-being, e.g.: nutrition, therapy support, medical assistance, job development, finances, needs, etc. Identify and resolve proactively potential concerns or adverse situations Identify obstacles or problematic behaviors Provide conflict resolution between home sharers Create case management that is proactive and supports each unique client and household Create regular check-ins with proactive communication and practices to ensure SHARE maintains a low attrition rate; that each Community House is successful Monitor and document through case notes the progression/stages of each matched client Monitor and document weekly, bi-weekly, monthly, etc. Provide remedial solutions Proactively address the impact on clients when any changes occur within each Community House Document client improvements or declines / continued upstream / stabilization of each client Develop and monitor agency referrals and collaborations with service providers involved with shared clients Identify each agencys timeline for collaborative support of mutual client Help identify and establish boundaries and expectations of each client Partnership with Sonoma County Enhanced Care Management / Behavioral Health Division Attend pertinent new placement rental agreement signing Schedule regular client and Community House check-ins Identify and coordinate the support system for each client Continue to improve own competencies through educational resources Identify and create educational / training opportunities for self, read technical publications relevant to SHAREs work, join professional networks, and participate with peers in other agencies Recommendations to Executive Director for improving SHARE results through studying, evaluating, and re-designing internal processes Collaborate with Executive Director to further develop Case Management policies and procedures SHARE tends to serve more vulnerable populations of Sonoma County. Experience in working with clients who are chronically unhoused and/or have mental health diagnoses, those over 50 years of age, and also households that include children is preferable. Required Skills [Experience in supporting chronically un-housed clients as well as those diagnosed or un-diagnosed with a behavioral health concern. Ability to meet each client where he or she is at without judgment, condemnation, or discri ination Ability to communicate in a non-judgmental and unbiased manner Compassionate listening skills Ability to maintain a can-do attitude; remain flexible and adaptable High-level energy; efficient work ethic; goal-oriented Ability and willingness to work in a team environment and provide a positive team spirit Must be a self-starter with a strong ability to work independently Ability to arrive on time and to maintain good work attendance Dress in a professional manner and uphold professional decorum at all times Demonstrate problem-solving and decision-making abilities Ability to maintain clear personal boundaries, and to practice good self-care Strong administrative, organizational, and time management skills Excellent intuition, compassion, patience, and listening skills Willingness to tackle a variety of tasks in a growing, fast-paced non-profit work environment EXPERIENCE Experience and ability to connect and work with those who are vulnerable, disabled, and of an older population (50 years and older), and households that include children Experience and ability to: Connect, work with, and support those experiencing homelessness or in danger of homelessness Experience and ability to support clients with a history of addictions, drug, and alcohol, opioid, gambling, etc. Experience and ability to support clients who relapse Experience and ability to establish boundaries to create positive outcomes and ensure the safety of each household/community Ability to identify and assess each clients level of care, vulnerability, resources, and services required Familiarity with community resources and services for the aging community Familiarity with community resources and services for those experiencing homelessness An understanding of the shelter system throughout Sonoma County An understanding of the workings of the Rapid Rehousing HUD funding program Proficiency in fair housing rules and regulations is helpful Bilingual a plus OTHER REQUIREMENTS B.A. or B.S. from a four-year college; minimally five years of training; or equivalent combination of education and experience, preferably in the field of Human Services, Psychology, and/or Gerontology Valid California drivers license and proof of current automobile insurance Competence in MS Word software, Excel a plus, Outlook, Google Docs, database entry (training will be provided), proficiency in email correspondence Must have a functional vehicle able to withstand 300+ miles per month May be required to lift up to thirty pounds Post-offer: Must be willing to undergo a background check and provide at least three work-related references Fully vaccinated for the COVID-19 virus is preferred due to client vul ]{arial",sans-serif"=""}
