TheRegistered Nurse(RN) Case Managerplans, organizes and directs hospice care with emphasis on community health education/experience. You will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. You will complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. You will complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.
