We are currently seeking an Youth Prescriber who will offer psychiatric assessment and medication management within the outpatient Youth Psychiatry setting on a full time basis. This opportunity is a day shift, no weekends, holidays or on call responsibilities. We offer a community based, empathic, compassionate health center atmosphere.
The Youth Prescriber is responsible for individual services to patients assigned by Crossroads Health. This licensed provider will engage as a member of a collaborative team. With a Trauma Informed approach, this position will be a representative for the organization and how we offer integrated services for recovery and mental health.
Additionally, the Prescriber will:
- Provide psychiatric assessments, medication evaluation and medication management.
- Regularly assess consumers & prescribe medications for clients via office and home visits
- Educate consumers & their families regarding medications/symptoms/illness/side effects. .
- Participate in ongoing assessment and therapeutic engagement. Complete, as directed, elements of the comprehensive assessment for clients within 60 days of admission.
- Master's Degree (minimum)
- Licensed in the State of Ohio as an NP/ PAC
- Controlled Substance Registration Certificate
- Medicare Provider Number
- Professional Liability Insurance.
- Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable)
- BLS/First Aid (employer provided)
