ESSENTIAL FUNCTIONS :

To help ensure that each participant remain stable at the site and make progress toward his permanency goals prescribed by the permanency order. This position will be responsible for performing day-to-day support services assistance to the participants of the program.

To manage all aspects of a participant's case (Court visits and court reports, ACR reports and visits, participant advocating, medical/dental visits, hospital visits, family attachment, etc.) as it relates to their residence and growth while at Garden of Prayer Youth Center.

ORGANIZING AND COORDINATING :

Screening/Intake

1. Conduct intake process and intake meetings with therapist, DCFS worker, and participant.

2. Assure the retrieval of necessary documentation for participant admission into the program.

3. Conduct participant orientations to familiarize them with program expectations, conduct clothing inventory, and build rapport.

4. Conduct the process of receiving new participants.

5. Ensure 906 forms are completed promptly.

6. Some duties may be reassigned and other duties may be asked of you as required.

Interfacing - Communication

1. Maintain proactive, effective communication with participants, direct care staff, therapist, and Program Coordinator regarding all participant issues and concerns.

2. Maintain proactive, effective communication with participants, participant family, DCFS workers, DCFS Monitor, DCFS Licensing Monitor, Program Coordinator, GAL, and all other stakeholders involved with the participant's case.

3. Ability to observe participant behavior and participation, assess appropriateness of such, and prepare reports and other correspondence in order to advocate and communicate all pertinent information regarding participants.

4. Build positive rapport with participants, colleagues, all DCFS personnel, and all other stakeholders involved with participant's case.

5. Set a good example for participants including cleanliness, punctuality, sportsmanship etc.

6. Maintain positive public relations with participant's families.

7. Facilitate interaction between participant, and participant as well as participant, and staff.

8. Participate in department and company employee meetings.

9. Provide frequent feedback and positive reinforcement to participants.

10. Interact and effectively communicate with colleagues in a professional fashion.

11. Communicate all necessary information to CCI Administrator and Direct Care Worker Supervisor

Linkage

1. Serve as participant's advocate to court system, school system, etc.

2. Manage all participants' medical/dental, psychiatric, appointment and ensure they remain current.

3. Manage all participants' ACRs, court visits, school enrollment.

Monitoring

1. Provide added supervision of participants to milieu.

2. Monitor all case management issues for participants and ensure that all documentation and visits are current.

3. Monitor participant campus attendance.

4. Monitor and maintain participant visitation plans and procedure.

5. Monitor the procurement of drug screening of participants by staff.

Direct Service

1. Attendance at all scheduled court and Administrative case reviews when required

2. Assist with completing the 497 (service plan) according to DCFS policy and ensuring that the youth is in attendance at the case review

3. Assures that the youth attends the scheduled CFTM's, CIPP and other meetings for purposes of planning, when necessary

4. Monitors the youth progress, academically. This includes but is not limited to school/vocational enrollment, collecting and maintaining school records and documents, collaborating with school staff and attending school conferences

5. Maintain client case file in AP-5 order

6. Complete and/or make sure that UIR's are written, distributed and maintained

7. Assure that the client has access to and meets with GAL and all legal representation

8. Assure that all court, probation or parole orders are followed and adhered to

9. Arrange sibling or parental visits if appropriate and upon request and document all visits

10. Assure youth is at all court appearances when attendance is mandatory

11. Weekly visitation to youth when hospitalized or incarcerated and provide youth with allowance

12. Contacting SASS for an evaluation/screening in the event that a youth is psychiatrically hospitalized

13. Obtain all necessary consents

14. Significant Event Report

15. All other duties as assigned according to DCFS policy

ADMINISTRATIVE :

Documentation/Record Keeping

1. Prepare and submit all documentation and reports accurately in a grammatically correct fashion.

2. Documentation of any Unusual Incident of any participants on a UIR Form.

3. Submit all necessary documentation to Program Coordinator, HR Dept., DCFS personnel and Executive Director as required

4. Some duties may be reassigned and other duties may be asked of you as required.

Agency Activities/Trainings/Other

1. Attend and participate in all initial and on going training

2. Actively participate in orientation and staff training, assigned staff meetings

3. Attend "All Staff" meetings

Requirements

QUALIFICATIONS :

Education: Requires a bachelor's degree in social work or a related human services field from an accredited college or university

Experience: 3 years experience working with at-risk youth. Experience with domestic violence, substance abuse and sexual abuse

Credential: Requires DCFS licensure as a child welfare specialist

Candidates must have a valid Illinois driver's license, good driving record, and a clear background.

Required Knowledge, Skills and Abilities

1. Ability to communicate and work with children with varying ages and skills; and provide necessary guidance, support, and supervision to participants at all times.

2. Mathematics and language arts proficiency

3. First aid and CPR certified

4. Passion to make a positive impact on the lives of youth in the community and to educate them to proficiency

5. Ability to observe participant behavior and participation, assess appropriateness of such, and enforce safety regulations and emergency procedures.

6. Visual and auditory ability to identify and respond to environmental and other risks related to an activity.

7. Physical endurance to engage children and adolescents in respective activities

8. Physical strength and ability to respond appropriately to situations requiring medical intervention, and de-escalation.

9. Cognitive ability to provide leadership to achieve participant goals and objectives.

10. Excellent communication and organizational skills

11. Able to work collaboratively with teams

12. Demonstrated reliability regarding attendance and work performance

13. Ability to be creative, flexible and adaptive in ALL situations, activities and responsibilities

14. Must be a team player

15. Must want to help address the needs of the community
