RN Health Coach

As an RN Health Coach, you will provide health coaching and care management services to Oklahoma Medicaid members in the statewide Health Management Program. You'll maintain a caseload of Medicaid members with chronic conditions or special health needs. You'll complete healthcare screenings and assessments for individuals with healthcare needs by collecting medical and social information to assist in the development of a person-centered care plan. You'll collaborate with healthcare providers, provide education and information to members, and make referrals for statewide community resources.

Hours are 8:00 am to 4:30 pm Central Time, Monday through Friday.

Essential Functions
    • Conduct comprehensive assessments and evaluations for individuals with diverse healthcare needs, gathering pertinent medical and social information to facilitate the development of patient-centered action plans.
    • Foster collaborative relationships with healthcare providers to ensure coordinated care delivery.
    • Provide evidence-based education and information to members, enhancing their understanding of health conditions and treatment options.
    • Identify and coordinate referrals to appropriate statewide community resources, addressing transitional care and social determinants of health.
    • Actively engage patients and their families using motivational interviewing techniques to promote behavior change and improve health outcomes.
    • Teach self-management skills and provide ongoing support to members and their families, empowering them to take an active role in their healthcare journey.
    • Utilize a holistic approach to care, considering physical, emotional, and social factors that impact overall well-being.
    • Maintain accurate and timely documentation of all patient interactions and interventions in compliance with organizational and regulatory standards.
Requirements
    • Registered nurse in Oklahoma is required.
    • Two or four-year degree in health care or a related field.
    • 3+ Years of experience.
    • Ability to speak Spanish is preferred.
    • We'd love it if you had experience with motivational interviewing.
    • This role requires excellent communication and organizational skills as well as proficient computer skills.
    • Extensive experience in disease management and case management skills.
    • Demonstrates proficiency in documentation and possesses strong writing abilities.
    • Self-motivated professional with a proven track record of delivering high-quality patient care.
    • Pediatric complex care experience preferred.
