Position Details:

We are seeking an experienced Advanced Practitioner for our REACH Program serving geriatric patients in the greater Fairfield area.

Role: Open to Nurse Practitioners (FNP, ANP, AGNP) and Physician Assistants (PA)

Location: Homecare based in the greater Fairfield, CT area

Setting: Geriatric Medicine in the home setting, working within a multi-disciplinary care team

Schedule: Full-time, weekdays

Role Scope:

Under established guidelines, Advanced Practitioners provide comprehensive care to geriatric patients in a home setting, working collaboratively with the care team and referring to specialists as necessary.

Qualifications

Requirements:

• Licensure: Unrestricted State of Connecticut Medical License (or eligible)
• Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
• Experience: At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
