MDS Coordinator

Position Available: MDS Coordinator Part-time The Steven and Alexandra Cohen Pediatric Long-Term Care Pavillion at Blythedale Children's Hospital is seeking an MDS Coordinator for our 24-bed facility. This is an exciting part-time opportunity for an experienced MDS Coordinator to make an impact on the delivery of medical care to children and families who are challenged by complex medical issues. The successful candidate will have an established reputation for leadership, teamwork, and partnering with residents and families. Responsibilities: Overseeing accurate and thorough completion of the Minimum Data Set (MDS), care area assessments (CAAs), comprehensive care plans (CCP), and interim care plans, in accordance with federal and state regulations and guidelines. Participating in regular care planning meetings with the interdisciplinary team (IDT). Ensuring all new admissions and readmissions are evaluated within 24 to 72 hours for Medicaid eligibility and/or significant changes in medical status and reporting such changes to the Director of Nursing. Monitoring the Case Mix Index scores and CASPER report for potential risks, while ensuring compliance with regulatory reporting and guidelines. Requirements: Bachelor of Science Degree in Nursing required. Licensed, or eligible for licensure by the New York State Department of Education as a registered professional nurse. A minimum of 1+ years of MDS experience or 3+ years of Long Term Care experience. Pediatric RN experience is required. BLS certified, PALS encouraged. Must exhibit fine motor coordination in the manipulation of biomedical equipment (e.g., thermometers, scissors, etc). Salary Range: $49.00 - $55.00 per hour.
