We are seeking a dedicated and passionate Unit Supervisor (LPN or RN) to lead the delivery of exceptional care to our residents in the Alzheimer Care Unit. In this role, you will independently oversee resident care, assess individual needs, develop tailored care plans, administer nursing services, and evaluate outcomes. You will also supervise a team of Nurses and Care Specialists (CNA), ensuring comprehensive support for our residents.

Key Responsibilities:

  • Ensure the delivery of effective, quality nursing care that focuses on desired outcomes through the nursing process.
  • Collaborate with residents and the interdisciplinary care team to develop individualized care plans.
  • Implement care plans consistently and efficiently, emphasizing resident-centered outcomes and utilizing available resources for counseling and discharge planning.
  • Maintain optimal resident care and comfort by demonstrating current nursing skills and practices.

Qualifications:

  • Must possess and maintain a valid license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the practicing state.
  • Current CPR certification is required; licensed nursing staff must obtain certification within 90 days of hire.
  • Recent clinical experience or specialized skills in geriatrics is preferred.
  • Able to maintain regular attendance.
