Job Description

The Holistic Defense Case Management Supervisor is responsible for supervising the Holistic Defense Case Managers. This Supervisor works closely with the Director to report and to resolve complex issues and works with the assigned staff to ensure compliance of all program protocols. This position must be filled by a dedicated individual who is social justice oriented, a team player, and committed to working alongside the legal and social team, clients served and their families as well as the community partners. The Supervisor reports directly to the Director of Holistic Social Services.

Responsibilities
  • Supervise and support Case Management staff.
  • Ensure Case Managers are performing their job duties including creation of service plans and intervention strategies.
  • Auditing and reviewing client records to identify corrections to be made and any other issues that may arise in providing services to the client.
  • Providing training to new case managers on department policies and procedures
  • Coordinate assignment of cases.
  • Providing support and advocacy for clients by helping them obtain services such as education, job training, employment, housing, counseling and other clinical services.
  • Maintain accurate documentation of service objectives and outcomes as well as other services in accordance with organizational guidelines.
  • Maintain related administrative record keeping and reporting systems required by contracts.
  • Attend program-related community, coalition and committee meetings as assigned. Participate in community presentations to promote the agency programs in general.
  • Create and implement programming centered around contract deliverables.
  • Manage grants in conjunction with the Development department.
  • Other duties assigned.
Knowledge, Skills, and Abilities
  • Self-starter who is passionate about supporting underserved youth.
  • Ability to lead in a fast-paced, team-oriented environment.
  • Ability to manage and perform in high stress situations.
  • Management or supervisor experience.
  • Ability to work independently and effectively manage the team.
  • Ability to collaborate with an interdisciplinary team.
  • Individuals from, or with connections to, North Lawndale or adjacent neighborhoods will be given special consideration.
  • Willingness to live in North Lawndale preferred.
Required Education and Experience
  • Master of Social Work, Counseling or related field preferred or other related experience.
  • 2+ years’ experience engaging and building mentoring relationships with court-involved and/or high risk youth and young adults.


Work Environment/Travel

While performing the duties of this job, the employee works mostly in the field (including home and school visits) with some occasional work in an office environment that is quiet to moderately loud, depending on the work area. Some local, county, or state travel may be required, including overnight stays and the suburban Cook County area. Other travel of distance (may be required. Must possess and maintain an applicable Illinois Driver License, clean driving record, and auto insurance that meets state requirements.

Physical Demands

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is required to stand; walk; sit; use hands to finger; reach with hands and arms; climb and descend stairs; balance; stoop, kneel, crouch or crawl; talk or hear. The employee may occasionally be required to lift or move up to 25 pounds. Requires normal range of motion, hearing and vision. The employee will spend 60% - 70% of the time in the field with clients and with partners. The employee will spend 30% - 40% of the time in the office or remotely supervising staff and handling administrative duties.

Position Type/Expected Hours of Work

It is a managerial level position requiring the exercise of discretion and independent judgment and is required to work a full-time work week, as designated by the LCLC Employee Handbook, with periodic longer hours, including potential evenings, weekends, and odd hours.
