As a full-time practitioner, The selected candidate will perform comprehensive evaluations of elderly patients, including their physical, cognitive, psychosocial, and functional abilities. They will also supervise the monitoring of vital signs and the management of medications.

Conduct in-person assessments and provide ongoing medical care to participants.
Work with doctors and the interdisciplinary team to create individualized care plans for each participant.
Work with doctors and the interdisciplinary team to plan for discharging patients from acute or long-term care settings.
Assist in providing care to nursing home residents by making regular visits based on their needs.
Collaborate with the doctors, community-based PACE doctors, and the interdisciplinary team to oversee and ensure effective care coordination.
Based on the direction of the medical director, Analyze outcomes and utilization data, such as hospitalisations and specialist use, to establish the best practices and improve policies.
Candidate requirements
An up-to-date resume.
A Master's degree in nursing is required.
A minimum of two years of senior population experience is required.
An active, valid registered nurse license within the jurisdictional region is required.
Up-to-date knowledge of chronic care and geriatric issues, as well as best practices, is required.
A valid state driver's license and a clean driving record from the Department of Motor Vehicles(DMV) are required.
A recent copy of your vehicle insurance is required.
Proof of COVID-19 vaccination is required.
Have communication and collaboration skills for working with teams from different fields.
Must be willing to travel at all times.
