Your Role in Hospice Care

As a Hospice RN Case Manager, you will work alongside an interdisciplinary team to assess, plan, implement, coordinate, monitor, and evaluate the comprehensive care needed for patients and their families during this crucial time. You will develop and adjust a personalized care plan based on the unique changing needs of the patient and family, ensuring a serene and dignified death experience.

Responsibilities

Reporting directly to the Nursing Manager, you will utilize your clinical judgment to implement effective treatment plans, monitor patient conditions, and prevent complications associated with health issues and medical treatments.

Qualifications

  • Associate's Degree in Nursing (required).
  • Bachelor's Degree in Nursing (required within 5 years).
  • Current MI RN License (required).
  • Preferred: At least 1 year of experience in acute care, emergency, or rehabilitation settings.
  • AHA or ARC Basic Life Support (BLS) certification required within 90 days of hire.
  • Valid Driver's License in the State of Michigan.

Primary Location: Continuing Care - 1239 E Beltline - Grand Rapids

Department Name: Skilled Nursing - Hospice

Employment Type: Full time

Shift: Day (United States of America)

Weekly Scheduled Hours: 40

Hours of Work: 8:00 a.m. to 4:30 p.m.

Days Worked: Monday to Friday

Weekend Frequency: On-call weekends
