On Call Triage Nurse

Are you an RN who has a passion for hospice care? Maybe you have family obligations during the day and you are missing nursing, helping patients? Do you think you could take patients calls, assess their clinical needs and if needed able to go out to their home during the hours from 5pm to 8am Sundays and Mondays? While the majority of calls can be handled over the phone there are times when the patient will need to see you. Flexibility is needed as Initial training will be weekdays. This position could be ideal for the right person Position requires flexibility to help in holiday rotation.
Preferred minimum qualifications include one to two years of nursing or triage, oncology, hospice, palliative care or med/surge experience. We are willing to train Must be able to work independently, have excellent communication, strong clinical and assessment skills. BSN preferred, will consider Associates RN with relevant experience. NH RN license and CPR certification required. EPIC EMR a plus

Service Areas: Southern New Hampshire.
Responsibilities:
- Triage after hours according to established protocol
- Making arrangements for supplies, DME and medications for patients with goal of positive patient outcomes and reduction of preventable ER/Hospital admissions.
- Contact patient's physician and/or hospice medical director to report any changes in patient's status and/or to obtain further orders
- Makes patient visit as necessary, admission or regular visit
- Coordinates the after hours healthcare needs of the patient by utilizing a disease management process, critical thinking skills, sound judgment, physicians orders, plan of care and established protocols
- Enter orders into EPIC as needed
- One to two years of nursing or triage, oncology, hospice, palliative care or med/surge experience.
- Associate degree in nursing.
- Registered as RN with New Hampshire's Board of Nursing.
- A minimum of one year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- EPIC experience preferred, willing to train
- Bachelor degree in nursing.
- Community Health/Education experience.
- Certification in Hospice and Palliative Care, Oncology or end-of-life experience is preferred, but willing to train the right candidate.
- EMR software proficiency.
- Registered as a RN with New Hampshire's Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
