Alamo Supportive Care is seeking a PRN Palliative Care Advanced Practice Nurse to provide compassionate care and support for patients facing serious illness. This role will involve delivering treatment and follow-up for patients referred to palliative care, conducting home visits within a 60-mile radius of our San Antonio, TX office, and collaborating with the Palliative Care Medical Director and other team members. Candidates must be licensed as a Nurse Practitioner in the state of practice and possess a minimum of two years of varied work experience as a professional nurse. If you are dedicated to providing comfort and goals of care conversations for patients with a serious or terminal illness, we encourage you to apply.
- Deliver compassionate palliative care to patients
- Conduct home visits within a 60-mile radius of San Antonio, TX
