POSITION SUMMARY

The Dietitian/Nutritionist completes a variety of professional assignments to improve the diet, nutrition and health of the home health patients serviced by the agency. Provides counseling to patients with medical problems affected by nutrition status and nutrition education to patients with a focus on disease.

QUALIFICATIONS

  • Graduation from an accredited college or university with an advanced degree in nutrition or its equivalent in education / experience or a bachelor's degree in nutrition with licensure as a registered dietitian; or any equivalent combination of training and experience.
  • Ability to communicate ideas effectively: orally and written.
  • Thorough knowledge of the principals and practices of nutritional care and counseling.
  • Ability to identify nutrition education needs and to develop an appropriate program to meet the individual patient needs.
  • Ability to maintain favorable public relations.
  • Possesses and maintains current CPR certification.
  • Reliable, independent means of transportation to make home visits. Drivers must have a valid driver's license and drive a vehicle with insurance limits as established by the state
DUTIES AND RESPONSIBILITIES

Clinical Skills

  1. Provides nutritional counseling on an individual basis upon referral by clinicians as a part of the clinical Plan of Care
  2. Instructs patients, families and health care staff in individualized diet therapy, general nutrition principals and food selection and preparation
  3. Conducts nutritional screenings/assessments for assigned patients
  4. Acts as nutrition resource person on the health care treatment team
  5. Plans individual menu for patients to meet nutritional needs
  6. Develops and maintains print and non-print educational materials on nutritional topics for use by clinicians and patients
  7. Provides in-service educational programs for clinicians on nutritional topics as needed
  8. Maintains patient and staff confidentiality.
  9. Handles general inquiries whether received by phone, mail or in person
  10. Performs related work as required

Communication
  1. Maintains ongoing liaison and communication among the staff
  2. Reports to the primary nurse/therapist about the patient situation including:
    1. Problems concerning the patient and how these may be resolved
    2. Changes in patient needs and how these can be met
    3. Patient progress toward goals and functional status
  3. Communicates with other health team members to achieve interdisciplinary coordinated care
