Orthodontist

United Dental Partners is seeking a full or part-time Orthodontist for our practice(s), located in Elgin/Crystal Lake, IL . The opportunity includes generous base salary, plus bonuses. This practice is a PPO, Medicaid (20%)/Fee for Service practice.

Requirements

• DMD or DDS from accredited dental school
• Specialty license in Orthodontics
• Valid IL state dental license
