Position Highlights:
  • Flexible Schedule: 9am-5pm; rotational weekend calls every 6-8 weeks.
  • Patient-Focused Role: Be a beacon of comfort and support for patients and their families on their hospice journey.
  • Collaborative Care: Work closely with a supportive interdisciplinary team to enhance patient care and resource utilization.
  • Experience and Qualifications: Hold an Illinois RN license, and a BSN is advantageous. Bring at least one year of recent acute care/hospice experience and a strong dedication to the hospice philosophy of care.
Job Description:

As a valued member of our Hospice Care team, you'll receive patient assignments from our Hospice Operations Leader. Your primary goal will be to foster a positive work environment through effective communication while handling essential duties such as assessing, planning, implementing, and evaluating patient care. Collaborating with attending physicians, hospice staff, and other agencies will be essential for ensuring optimal care and resource utilization. Your Responsibilities Will Include:
  • Maintaining thorough patient records to ensure clarity and accuracy in the care process.
  • Exhibiting unwavering dedication to the hospice philosophy of care, supported by at least one year of recent acute care/hospice experience.
Requirements:
  • Current RN licensure in Illinois.
  • Preferably possess a BSN.
  • Proof of auto insurance covering bodily injury and property damage.
