Job Title: Case Manager Assistant Job Description Join our dedicated care coordination team as a Case Manager Assistant where you will be pivotal in executing transition of care plans. You will collaborate with Case Managers and Social Workers in assessing patient needs post-hospitalization, developing and communicating the transition of care plan, and ensuring smooth discharge processes. This role involves monitoring the progress of care goals, coordinating post-discharge referrals, authorizations, and ensuring all involved parties including the patient and medical facilities are informed throughout the hospital stay. Your contributions will directly impact patient outcomes by facilitating timely transfers to post-acute facilities and addressing any barriers to the transition of care plan. Required Skills 1 year of relevant medical experience EPIC EHR experience required High School Diploma or equivalent Effective communication Proactive problem-solving Collaboration Respectful customer service Ability to handle confidential information Job Type This is a contract position with a duration of 26 Week(s). Work Site This is a fully remote position. Work Environment Fully remote position with a flexible schedule that includes day shifts, rotating weekends, holidays, and possible evening shifts. Employees from different states will work hours based on their time zone, not necessarily CA time zone hours.
