HealthPartners Well@Work Clinic is looking for a Nurse Practitioner or Physician Assistant to join our team Well@Work is an employer based clinic supporting the personal health needs of the client's employee and dependent population. In addition, this role partners with the client to support various Occupational Health needs such as pre-placement exams, DOTs, and work comp care. A current DOT certification or ability to obtain after hire is required. The Advanced Practice Clinicians at North Suburban Family Physicians identify and manage healthcare of individuals and families (consistent with description of practice certifying specialty). The Nurse Practitioner/Physician Assistant works autonomously as well as in collaboration with a supervising physician, according to North Suburban Family Physicians guidelines. Other medical activities include physical examination, history taking, developing comprehensive medical status reports, formulating a tentative medical diagnosis and initiating therapy under medical supervision. Care provided emphasizes health promotion and disease prevention, as well as diagnosis and management of acute and chronic diseases. The Nurse Practitioner/Physician Assistant encourages patients to be responsible for their own health behavior through education, support and involvement in their plan of care. Nurse Practitioner Required Qualifications: Graduate of an accredited school of nursing and formal nurse practitioner program. Masters' degree is required for candidates graduating after 1992. Current RN License, Certified or eligible for certification through ANA national exam as ANP, FNP. If without certification, must pass certification exam within 12 months of hire. Approved or eligible for approval by the Minnesota Board of Nursing for prescription writing privileges. Current CPR certification. May provide service to patients at multiple clinic locations. Must be able to provide own transportation for local travel. One-year experience as ANP, FNP preferred. Active member of a professional nursing association preferred. Physician Assistant Required Qualifications: Graduate of an accredited formal physician assistant training program. Current Minnesota PA license. Current certification or eligible for certification through NCCPA national exam as PA. If without certification, must pass certification exam within 12 months of hire. Approved or eligible for approval by the Minnesota Board of Medical practice for prescription writing privileges. Current CPR certification. May provide service to patients at multiple clinic locations. Must be able to provide own transportation for local travel. One-year experience as PA preferred. Active member of a professional PA association preferred. Physical Requirements: Lifting of infants/children and/or adults may be required. No other lifting requirements are identified, but the potential for lifting exists. Performing physical examinations, communication with parents/patients and i plementing medical or nursing treatments requires adequate vision, hearing and tactile motor skills. Must possess speech, hearing and sight skills to perform the functions of the position. Must be able to speak, read and write English proficiently. Body mobility -- coordinated dexterity of hand, ear and eye, sensory acuity and verbal ability to perform the functions of the position. Ability to work walking, standing, and/or sitting for prolonged periods of time. Ability to lift, carry, bend, reach, squat, kneel on an occasional basis. Ability to read, analyze and collect data. General Practice Guidelines: MN Board of Nursing sets the definition for nursing practice and licenses the NP practice. Commission on Education for the Physician Assistant (AAPA) defines the scope and protocol of the PA practice. National Certification Organizations such as: ANCC or NCCPA certifies the NP/PA specialty practice. The Nurse Practitioner/Physician Assistant Understands and accepts role to staff, and NSFP patients and members, maintains professional identity within the framework of the care team, develops and maximizes Nurse Practitioner/Physician Assistant Role and assesses the health status of individuals and families as appropriate. The Nurse Practitioner/Physician Assistant: Assesses the health status of individuals and families as appropriate. A complete health history is obtained and documented. Patient's perception of problem is elicited. A complete, accurate physical examination is performed and documented. Appropriate screening and diagnostic tests are ordered and evaluated. A problem list is developed. Wellness and risk factors are identified. Identify variations from norms and diagnose common health problems and diseases. Develops and implements plan of care: Sets goals and plans mutually acceptable to patient and provider. Prescribes a variety of medical treatment to meet wellness and illness needs, includes but not limited to medication, nutrition, exercise and traditional nursing treatments. Manages wellness and illness needs according to guidelines. Teaches and supports positive self-care activities including problem solving and selection of treatment alternatives. Provides health counseling and emotional support throughout care. Assists patient with health maintenance activities. Chooses interventions which will promote the patient's participation in his/her own care to the greatest possible extent. Evaluates the process and outcome of a management plan and modifies the plan as appropriate. Treats common childhood diseases. Assists in the follow-up of geriatric disorders referred by physicians. Prescribes medications for symptoms and temporary pain relief (if eligible). Performs routine therapeutic procedures (i.e., injections, debridement, foreign body removal from skin surfaces, removal of sutures, casting or splinting of sprains, I & D of superficial skin infections) as identified in the supervisory agreement referenced above.
