Summary:
Under the supervision of the Medical Director, the Hospice & Palliative Care Nurse Practitioner is responsible to provide hospice and palliative care to patients and their families, including assessment, planning, implementing and evaluating care needs in accordance with accepted standards of practice which supports the philosophy, mission and purpose of Hospice and Palliative Care. This position also provides clinical support to members of Hospice and Palliative Care team and to other staff as requested by the Service Line Director, Hospice Leaders, or Medical Director.

QUALIFICATIONS:
-Registered Nurse Practitioner with current PA licensure and a Masters in Nursing with focus on Nurse Practitioner training and responsibilities from an accredited Nurse Practitioner Program required. DEA license and prescriptive authority.
-One year's experience in provision of relevant direct patient care required. Previous experience in hospice/palliative care/home care preferred.

• Proven ability to work within an interdisciplinary setting.
• Complies with and promotes accepted professional standards and best practice.
• Has demonstrated critical thinking ability, problem-solving skills in an interdisciplinary setting.
• Has effective communication skills.
• Understands hospice philosophy and issues of death/dying.
• Understands principles of pain/symptom management.
• Ability to manage time effectively and prioritize projects.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
