Description

Purpose: (General description and summary)

The Intensive Case Management (ICM) Mental Health Clinician (MHC) is responsible for developing, implementing, and monitoring services to adult individuals diagnosed with mental illness. Consumers served by the ICM MHC will primarily be individuals requiring more intensive treatment than can be provided in a traditional outpatient setting. Diagnosis scope will require skills in working with folks with high acuity, those experiencing houselessness, frequent hospitalizations, substance use disorders, and/ or incidents of heighten emotions or psychiatric status. This role requires extensive care coordination and crisis de-escalation skills. This position will spend time providing treatment in both community and clinical settings. The ICM MHC will work in a team setting for service delivery, with individuals internal and external to New Narrative. Trauma informed care and person first orientations are required in this role, as is someone who demonstrates positive regard while supporting individual dignity and autonomy.

Accountabilities: (Responsibilities of the job)

Case Management and Direct Therapeutic Services
  • Develop relationships with each persons on the caseload by meeting for case management on at least a weekly basis, with increased contact as necessary.
  • Carry a caseload of no more than 20 people, as required by IDDT fidelity standards
  • Assess the strengths and service needs of each consumer.
  • Develop individual master treatment plan in partnership with consumer.
  • Assist consumer in connecting to resources and services needed to achieve goals set in individual master treatment plan, including helping them with community navigation.
  • Monitor service delivery and coordinate care with internal and external stakeholders.
  • Ensure scheduling and attendance of prescriber visits for medication management and problem-solve with clients to ensure effective medical services.
  • Provide customer service to New Narrative stakeholders.
  • Provide individual skills training to support individuals in their ADL and IDL goals.
  • Provide individual counseling to support individuals in their stability and overall wellness.
  • Provide transportation as needed to/from appointments to ensure connection and access to resources, as well as skills training opportunities in the community, etc.
  • Provided advocacy and advocacy resources for individuals.
Crisis Intervention
  • Complete crisis plan for each person who may need access to emergency services.
  • Complete WRAP plans or take other proactive steps to minimize use of crisis services.
  • Contact hospital within one (1) working day for anyone on the caseload who is hospitalized.
  • Document hospital and ER utilization and assist with discharge planning.
  • Coordinate consumer engagement with alternative resources and providers as needed to reduce time spent in emergency services.
  • Carry crisis phone on a rotating basis (approximately one week out of 8)
  • Provide community de-escalation and crisis intervention to aid individuals in maintaining in lowest restricted levels of care.
  • Communicate crisis interactions and subsequent needs of consumer to treatment team.
  • Prompt completion of incident reports and documentation after crisis interactions.
Administration:
  • Complete all required assessment, reauthorization, and funding paperwork.
  • Document all consumer contact and actions made on behalf of consumers.
  • Complete services plans and reviews, LOCUS (or LSI), and Transfer Summaries.
  • Work with support staff to ensure clinical charts are complete and in compliance with federal, state and county rules.
  • Assist with preparation of reauthorization paperwork in a timely manner.
  • Adhere to productivity expectations of spending at least 55% of time providing direct, billable services to individuals or groups.
Utilization Reviews
  • Provide information as requested by the Quality Improvement Coordinator.
  • Complete all documents and observe timelines to ensure that charts are in compliance.
  • Participate in supervision as needed to review interventions and utilization.
Other General Expectations:
  • Attend and positively participate in required meetings.
  • Consistent communication with treatment team and involved external providers for consumers to ensure integrated consumer care.
  • Pursue ongoing development of skills and competencies. A minimum of 16 hours on-going education, of which 12 hours must be in area of mental health/dual recovery and four hours in cultural competency.
  • Follow all corporate policies, procedures, and protocols.
  • Adhere to any applicable ethics codes.
  • Maintain current Oregon Driver's License.
  • Adhere to a regular work schedule as set by direct supervisor.
  • Follow through on all directives and instructions.
  • Maintain security of all corporate interests.
  • Maintain CPR and First Aid Certificate.

Education/Certificates/Licenses/Registration:

Qualified Mental Health Professional with a Masters Degree in social work or related field is required. Experience working with similar populations effectively is strongly preferred.

Experience/Qualifications:

Spanish/English preferred. Skills necessary to work independently. Must have a firm understanding of individuals with severe and persistent mental illness, psychiatric medications and their use, administrative rules and other applicable laws and regulations, as well as a firm understanding of acceptable clinical practices and guidelines. Complete understanding of the DSM and the ability to compose comprehensive mental health assessments and initial service plans.
