Daily Responsibilities for Licensed Clinical Social Worker:
  • Provide Behavioral health case management services both in-home and telehealth virtual visits to geriatric patients
  • Develop treatment plans that connect members with medical, psychiatric, and addiction services and other community-based resources
  • Hybrid M-F, 8am - 5pm, no evenings, weekends, or on-call

Required Qualifications for Licensed Clinical Social Worker:
  • LCSW in the State of New Jersey
  • 3+ years post-grad professional experience as an LCSW
  • Based in Passaic County, New Jersey and have a NJ driver's license
  • Intermediate computer proficiency and EMR experience
  • Bilingual Spanish and English
