Social Worker II

Our Client is seeking a Social Worker for our to ensure that we continue to provide all patients with high quality, efficient care. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do.
Job Duties
- Work in collaboration with the case manager to identify potential barriers and quality outcomes
- Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planning
- Works within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.
- Master's degree in Social Work or Counseling Psychology required
- Florida license required (LCSW - Licensed Clinical Social Worker or LMHC - Licensed Mental Health Counselor)
- One year experience in discharge planning in a health care setting
- Current BLS (Basic Life Support) required (within 30 days of employment start date)
- Current CPI (Crisis Prevention Intervention) required (within 30 days of employment start date)
This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved)
