Hinds Hospice, one of the most well-known and well-respected nonprofit agencies in California's Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Full Time RN Case Manager to join our team.
The RN Case Manager is an experienced Registered Nurse who provides skilled palliative and end-of-life care in the community setting. The RN Case Manager is also responsible for promoting the philosophy of hospice, which redefines hope by focusing on quality of life and connection with loved ones during a patient's final days, rather than on curing a disease.

Qualifications
- Compensation $53 - $55 an hour

- Completed a professional nursing program accredited by the National League for Nursing (prefer completion of a baccalaureate program in nursing).
- One year experience in acute care or community health center - hospice care a plus.
- Current RN Licensure and CPR Certification.
- Valid driver's license, auto insurance, reliable transportation and a willingness to use personal vehicle in the course of employment.
- Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
- Available to work Tuesday through Saturday 8:00AM to 4:30PM or Sunday through Thursday 8:00AM to 4:30PM
