Opportunity

The Behavioral Health Manager provides clinical leadership of behavioral health and/or transitions providers within Mosaic Medical primary care community health centers. In addition to working as part of an interdisciplinary team to provide behavioral health services to a diverse population within a patient centered primary care medical home. They will consult with primary care providers and other clinical team members on mental health, behavioral medicine, and substance use disorders. This role supervises the members of the behavioral health and/or transition team, assignment will vary based on region and/or area of focus.

Responsibilities

  • Provides direct patient care at a minimum of 75% in a primary care clinic setting.
  • Works with members of primary care team to assess, consult, treat, and manage patients with substance use, mental health, and behavioral health concerns within an integrated behavioral health care model.
  • Assists patients and family, through individual, group, family, and/or couples brief interventions within the context of an integrated behavioral health model, teaching coping mechanisms, providing support and guidance.
  • Manage performance of the team, conduct monthly meet-ups and provide real-time and consistent coaching for all direct reports.
  • Partners with Director of Behavioral Health, and other System Leadership Team members in developing and integrating chronic pain and substance abuse services within Mosaic’s primary care practices.


Skills & Knowledge

Has the competencies necessary to: Identify precipitating events; Gather histories of mental, emotional and physical disabilities, alcohol and drug use, and past mental health services; Assess family, social and work relationships; Conduct a mental status assessment; Document a DSM diagnosis; Write and implement or supervise implementation of a treatment plan; Conduct and document a mental health assessment; Provide mental health treatment and rehabilitative services within ones scope of practice; Experience working with adult populations with addictions and complex issues. Execute evidence-based brief interventions. Ability to make quick and accurate assessments of mental health conditions related to substance abuse. Interest in gathering, tracking and presenting outcome data. Expertise in trauma-informed, culturally sensitive substance abuse and mental health treatment. Be knowledgeable of local and state resources. Be able to work with patients from infancy to geriatrics. Comfort with electronic medical records and charting. Flexible and able to assess and triage on the spot demands of medical staff.
