Supportive Housing Case Manager- Ansonia
POSITION SUMMARY: Helping families thrive by finding homes in safe, nurturing neighborhoods, achieving economic independence, developing healthy relationships, and providing their children with a stable and caring home environment. Working towards the goal of preservation and the reunification of families.
Accountable and responsible for the following results:
- Perform administrative functions when needed and as directed by the Program Supervisor and/or Program Assistant Director.
- Complete intake and client assessments, evaluating clients for possible admission to the program.
- Coordinate team review meetings with DCF Social Worker, Substance abuse treatment provider, and other team members/providers as identified.
- Provides advocacy, crisis intervention and referrals to counseling services to clients and their families, as needed.
- Develop and monitor compliance with care plan.
- Maintain up to date records, documenting client progress and submit reports to the case review team maintaining the confidentiality of all records.
- Work directly with DCF social worker to ensure the safety and stability of children including unannounced home visits and on-call coverage.
- Provide case management services to program participants. This may include coordination of the following services: substance abuse recovery, relapse prevention children's services, educational support for parent and/or children, parenting training, child care, child development training, life skills training, health education, educational and vocational training, financial management, job development, and placement training, transportation and home management training.
- Provide transportation to clients as needed.
- Coordinate lease up with Family Unification Program vouchers.
- Participate in community and program development as requested by Program Supervisor or Program Assistant Director.
- Provide and maintain safety for consumers and staff.
- Prepare monthly statistical reports and complete daily CAMIS data entry information.
- Participate in off-site meetings at partner agencies.
- Maintain records in accordance with policies and procedures.
- Maintain and enhance professional proficiency by participating in continued education and workshops designed to enhance skills and understanding of family violence issues.
- Attend weekly staff meeting, case conferences, weekly supervision and all BHcare, Inc. required in-services.
- Attend monthly meetings as well as other required attendance per contract
- Must have ability to work flexible hours as needed to meet program and client needs.
- Attends training sessions, workshops, seminars as requested by supervisor, or as needed for professional development, or as required for accreditation or licensing.
- Required to use own vehicle on company business, or to transport clients.
- Regular and consistent attendance is an essential function of this position.
- May perform other duties as assigned.

MINIMUM QUALIFICATIONS:
- Bachelor's degree required.
- Knowledge of child welfare, substance abuse, welfare, or supportive housing system.
- Knowledge of culture of addiction.
- Must be proficient in Microsoft 365. Must be able to navigate through electronic health records system (EHR).
- Bilingual ability a plus.
- Must be able to obtain and maintain the certified domestic violence counselor certification per CGS 52-146k before providing direct service to clients, followed with six-hour recertification yearly
- Must have and maintain a valid driver's license. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Organization's auto insurance plan.
- Must be able to obtain and maintain BLS (Basic Life Support) certification
