As a Hospice RN Case Manager, you will act as the primary liaison between physicians, patients, and their families, providing direct patient care services and empowering caregivers through education and support. Your Role: Reporting to the Administrator or Patient Care Manager Senior, you will maintain primary control and professional management of each patient, evaluate patient and family needs, and assist in preparing and executing the plan of care for each patient. Additionally, you will collaborate with an interdisciplinary group to provide comfort, peace, and dignity to patients and their families.
Requirements: RN license in the state of requested employment, 1+ years of nursing experience in hospice, home health, or related fields, current driver's license and automobile insurance, and current CPR certification.
