Essential Duties and Responsibilities
The Behavioral Health Clinician will provide mental health therapy services within a primary care-based clinic. This position requires the formulation of an individualized treatment plan which will be used in conjunction with the entire primary care team to develop a holistic care strategy for each client.
- Perform related duties as assigned by supervisor
- Maintain compliance with all company policies and procedures
- Education, Licensure and Experience Requirements:
Education: Master’s degree in Social Work (MSW) Psychology, Counseling or Doctoral Degree in Psychology from an accredited college/university- Required Licensure: LMSW (Licensed Master Social Worker), LLMSW (Limited Licensed Master Social Worker) or LP (Licensed Psychologist)
Required Skills:- Proficiency in MS Office (Word Excel and Outlook)
- Proficiency in PC and general office equipment (Fax, Copier, scanner)
- Ability to maintain good records and be able to complete reports via electronic record (EMR) or ECO as needed
- Excellent written and verbal communication and documentation skills
- Job Knowledge and Comprehension
- Person Centered Planning includes completion of the person-centered planning document that provides an overview of the person and details his/her hopes, dreams and desires.
- Individual Plan of Service includes completion of treatment goals, measurable objectives, assigned staff and applicable treatment codes.
- Consent and Enrollment includes completion of required consent and enrollment documentation.
- Documentation includes complete, accurate and timely completion of clinical and statistical records of staff activities.
- Documents every consumer and collateral contact in compliance with agency policy.
- Complete various administrative forms to assure accurate data collection for billing, fee setting, the management information system and other purposes in accordance with agency policy
- Organizing Workload includes prioritizing work assignments in a manner consistent with consumer and program needs while making the most efficient use of available work time.
- Discuss continuing care needs with multidisciplinary care team.
- Provide psychotherapeutic support/counseling to client according to DSM 5 guidelines
- Performs other job-related duties as directed by Program Director.
- Ensures the proper handling and compliance of confidential medical records (EMR)
- Maintains record request for Medical records from other sources.
- Follow-up planning for clients with post-acute transitional concerns
- Document assessment findings in SW outpatient assessment or SW progress notes within EMR
- Communicate material findings and tentative transition plan with care team (nurse, physician, case management etc.) via EMR or face to face
- Coordinate care with health plans care management staff
Productivity- Maintains a minimum of 60% of available time in direct service
- Comply with and adhere to department and NSO policies and procedures
- Participate in interdisciplinary care team meetings.
- Adhere to established quality and performance improvement standards.
- Work effectively with others to accomplish goals/resolve problems.
- Organize work well and uses time effectively.
- Maintain consistent work attendance.
- Participates in interdisciplinary care team meetings
- Does not disrupt operations by being habitually tardy or absent; works as scheduled.

- Ensure consumer satisfaction through process of monitoring, improving, and delivering excellence in program services.

- Comply with contractual and regulatory requirements, as needed.

- Perform other duties as assigned.
- Services will be provided in the hospital setting and patients’ homes, places of respite or other appropriate community settings. Work may require travel in an urban community. Required travel of 40-60% in local area.
- Works in an office and outreach capacity; uses a computer, telephone and other office equipment as needed, to perform duties.
- The noise level in the work environment is variable, due to frequent contact with consumers.
- Bends, stoops, lifts, carries, and reaches in order to file, lift/carry work equipment (laptop, paperwork), search for and retrieve records and documents.
- Manual dexterity and regular fine-finger and hand/wrist motions are required for operating a keyboard, writing and filing.
- Seeing/vision, talking/speaking and listening/hearing are continuously required.
- Frequently required to stand during working hours.
- Frequently required to sit, walk or drive.
- Must be able to operate an automobile to performing outreach job duties.
