Job Description

The mission of the Boston Public Health Commission (BPHC) is to work in partnership with communities to protect and promote the health and well-being of all Boston residents, especially those impacted by racism and systemic inequities. PROGRAM/DEPARTMENT DESCRIPTION

The Men's Health Initiative (MHI) operates out of the LifeCourse Unit (The LCU) in the Child, Adolescent and Family Health Bureau of the Boston Public Health Commission and is part the City of Boston's Community Violence Intervention strategy functioning in collaboration with community partners and the Mayor's Office of Community Safety. MHI is one of the primary components of the comprehensive blend of "Public Health and Public Safety" approaches, tasked with City-wide firearm related violence reduction efforts. The program offers case management and supportive services to individuals in the city and those who are either court involved, returning citizens, recovering from violent injury, and/or at high risk of gang or firearm violence.

Additional partners of MHI are Community Based Organizations (contracted and non-contracted), Boston Medical Center, the Boston Police Department's Office of Research and Engagement, the Boston Regional Intelligence Center (BRIC), the Commonwealth of Massachusetts Executive Office of Health and Human Services, Massachusetts Department of Correction, Suffolk County House of Correction, Department of Youth Services, among others. The roles and responsibilities of this position include facilitating and assuring that MHI functions as a meaningful and impactful partner in the City of Boston's Community Violence Intervention and reduction efforts.

DUTIES

  • Conduct neighborhood and institution outreach (including Department of Corrections, County Jail and House of Correction, DYS and Offices of Probation, Boston Public Schools) to identify and connect with youth and families in need of services as it relates to the implementation of the Men's Health Initiative.
  • Work as part of a team of case managers along with other Local, State and Community Based Organizations and partners to developmentally support clients who are in the community as well as those that are re-entering from custody. Support clients to prioritize short- and long-term needs.
  • Providing "client centered holistic", case management services which includes intake, needs assessments, development of Individualized Service Plans (ISP), case conferencing, provide information and referrals for individuals in need of services such as employment, education, behavioral health, housing, health, and other relevant stabilizing efforts that benefit the client.
  • Assure timely and accurate documentation of all case management services. Work independently and alongside a team of case managers to manage an individual caseload utilizing a strength based approach.
  • Screen potential clients for appropriate services.
  • Collaborate with community partners to aid in the continuity of care for service users.
  • Provide non-judgmental support, crisis intervention, conflict resolution, referral, and advocacy.
  • Maintain client confidentiality and anonymity where appropriate.
  • Maintain the confidential client data base, including collection and recording of accurate statistics.
  • Conducting follow-up with youth and families.
  • Attend developmental appointments with clients.
  • Attend court appearances with clients.
  • Attend meetings as required by the grant initiative.
  • Attend training relevant to the position.
  • Other related duties as assigned.
  • Passionate commitment to health equity and racial/social justice issues.
  • Knowledge of or willingness to learn about the role of public health in addressing racism, the social determinants of health, and inequities in health outcomes as well as strategies to advance racial justice and health equity.
Minimum Qualifications

MINIMUM QUALIFICATIONS
  • High School Diploma or GED required. BA/BS preferred but may be substituted by substantial related education and work experience.
  • 2-3 years' experience working with at-risk youth and/or gang members (preferred).
  • Minimum 2 years program experience in community outreach/social service/case management/education activities.
  • Strong conflict resolution skills.
  • Knowledge of Boston neighborhoods and the ability to closely interact with gang members and high-risk individuals to diffuse conflicts within specific SSYI areas.
  • Strong working knowledge of community resources and social services in Boston.
  • Demonstrated understanding of family systems and youth development.
  • Demonstrated ability to be punctual, organized, and able to follow through on assignments.
  • Ability to work under pressure and in crisis situations.
  • Excellent written and oral communication and interpersonal skills.
  • Ability to work individually as well as in a team environment.
  • Ability to follow direction, provide/receive feedback, and engage in pro-active problem solving.
  • Knowledge of or willingness to learn about the role of public health in addressing racism, the social determinants of health, and inequities in health outcomes as well as strategies to advance racial justice and health equity.
Preferred or Plus:
  • Computer literacy and experience using MS Office.
  • Valid Massachusetts driver's license.
  • 1+ years of demonstrated formal or informal social justice advocacy experience.


Lack of particular requirements relating to experience working with "proven-risk" individuals (as listed above), are also not an automatic bar to employment. All appropriate consideration will be given to candidates with relevant lived experiences, knowledge of the specific demographic, and direct service skills determined to be beneficial to the success, goals, and objectives of the program.

Shift

35 hours per week.

9AM to 5PM; Monday to Friday.

Evenings and weekends may be required.
