SUMMARY

ESSENTIAL DUTIES

  1. Uses independent judgment and discretion to conduct regular home visits to determine client needs and provides training, guidance and support to families. Participates in and remains available to assist in crisis intervention and to provide support and assistance to staff in resolving problems.
  2. Uses independent judgment and discretion to provide casework and support services to the families and children, including referral for individual and/or family therapy; arranges contacts, visits and referrals and coordinates with community services, pediatric and dental care, and other services as needed.
  3. Ensures all client files are accurately and completely maintained.
  4. Represents CII at DCFS, DMH and community meetings; promotes CII’s goals and philosophies; participates in community committees and networks; and actively collaborates with others.
  5. Establishes and maintains rapport and effective working relationships with children and families and works effectively within CII and social work community at large.
  6. Uses independent judgment and discretion to identify client’s needs and provide community referrals to client and others.
  7. Advocates on behalf of the client with other agencies, government programs and similar to help them receive needed services.
  8. Ensures accurate and timely completion of outcome and evaluation testing and reports required by the program.
  9. Develops and maintains effective working relations with other agencies, schools, community organizations, placement centers and other resources to improve services for CII clients.


ADDITIONAL DUTIES

  • As required, conducts intake interviews to gather information and completes required paperwork.
  • Continuously increases professional knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in in-service programs.
  • Develops and maintains knowledge of service standards and policies as stipulated by contract, licensing, and/or other governing bodies.
  • Other duties and special projects as assigned.
SUPERVISORY RESPONSIBILITIES
  • This job has no supervisory responsibilities.
EDUCATION & EXPERIENCE
  • Master's degree in psychology, social work, human development, or a related field required
OTHER QUALIFICATIONS
  • Possesses a valid driver's license and state-required auto insurance
  • Required to travel extensively (> 50% of the time).
  • This is a 40 hour/week role. Hours may vary and you may take some appointments between 5-8 pm.
  • Spanish / English bilingual required.
  • Strong ability to work with others towards a common goal.
  • Ability to handle confidential information.
  • Maintains patient privacy, including protecting the confidentiality, safeguarding the integrity and availability, and limiting the use and disclosure of patient Protected Health Information (PHI) to authorized individuals and entities.
  • Participates in, conducts, and sustains performance quality improvement activities according to designated role and responsibilities.
  • Sensitivity to service population’s cultural and socioeconomic characteristics.
  • Excellent verbal and written communication skills.
  • Punctual and dependable attendance.
  • Free from alcohol and drug abuse.
  • Understands CII’s basic philosophy and participates fully in carrying out its mission.

COMPENSATION:
  • Hourly hire rate: $26.75
