We’re excited to offer newly enhanced, highly competitive, sign-on bonuses to experienced Social Workers with at least 12 months of experience Apply today to learn more Lowell General Hospital’s well-respected Continuity of Care Team is looking for a Social Worker to join their team This team consists of incredible professionals with great energy all working towards putting patients first in everything they do Come work alongside a dynamic, compassionate, and resourceful group of Social Workers where you will feel appreciated, and supported You’ll have a great work-life balance with day shifts and limited weekend rotations, plus no on-call scheduling Hours: 40 hours/week First Shift- Flexible Hours, part-time roles also available Limited weekend rotations Duties & Responsibilities: As a Social Worker here at Lowell General, you will: Asses selected cases within a defined patient population to evaluate for potential psychosocial barriers to quality patient outcomes and timely, safe, and appropriate discharges Work with individuals who may have substance abuse, mental health conditions, or housing instability Intervene in situations where the treatment plan can be significantly impacted by providing the patient/family with support and counseling regarding social, emotional, and/or financial issues. Work collaboratively with a healthcare team Focus on connecting patients and their families to resources in the community Assist hospital staff dealing with cases of abuse and neglect and report cases to appropriate agencies/authorities.
