DescriptionGeneral Responsibilities: Provide ongoing coordination and collaboration of case management services, therapy, treatment planning, resource identification and development, advocacy, service monitoring and follow-up, collateral supports in and outside the organization, and treatment services as appropriate to assigned caseload including documentation of the same under the supervision of a Mental Health Professional.
Essential Functions: (Tasks, Responsibilities, and Competencies. May not include all duties of this job)
- Monitors illness management and resource needs of assigned clients.
- Assists and advocates for the client in the following areas:
- Crisis needs: Monitor client behavior to predict and avoid crises. Notify appropriate professionals, team members and CDMHPs, of client status, coordinate with service providers (hospitals, evaluation and treatment center, and crisis beds) to ensure client safety and monitor relapse prevention activities and strategies.
- Ethnic/Cultural support: Assist client as needed to connect to these supports and seek required and/or appropriate consultation for minority populations.
- Physical health care needs: Refer client to physician to address physical care needs, assist client in attending appointments. Assist the client to inform the prescriber for psychiatric med management services of their physical health care plan.
- Psychiatric services: Help schedule psychiatric services as indicated, follows prescriber's directed treatment, monitors medication compliance and provides psychiatrist with ongoing information (as indicated) regarding client status.
- Delivers services as stated.
- Seeks continuance when indicated.
- Completes all required documentation in a timely and thorough manner consistent with approved practices, policies and procedures.
- Functions as the consumer liaison/advocate in relation to the mental health system, community agencies and the consumer's support system as needed.
- Provide timely and accurate compliance with all required and appropriate charting and coding as required by the regulatory organization (Medicare, MCO, etc.).
- Attend all training required by Sunrise.
- Maintain all on-going licensing and certification requirements for this position and supply Sunrise proof of the same.
- Develop expertise in solution focused and cognitive behavioral therapy and group process, provide this treatment, and share this knowledge with other staff.
- Communicate with consumers and family members to obtain information regarding consumer's needs and objectives, interests, skills, and support systems.
- Maintain consumer records in keeping with agency, county, Managed Care Organization (MCO) and state guidelines.
- Act as a backup for other team members.
- Prompt response to client needs.
- Support and encourage recovery model.
- Utilize clinical supervision for clarification of questions and concerns, support, understanding role expectations, feedback and accountability.
- In addition to the above, any other responsibilities appropriate to the position and not specifically listed in this job description or tasks that may be assigned on an as needed basis.
- Perform other duties as assigned.
- Education: Pursuing a APRN credentials at an accredited college or university
- Experience: Possess nursing experience
- Licensure/Certification: Holds a current Washington State Nursing licenses
- Other Considerations:
- Current valid Washington State driver's license.
- Excellent oral and written communication skills
- Able to ensure and deliver client care and provide a legal record of the care.
- Superior interpersonal skills and ability to work with difficult clients.
- Demonstrated respect for diversity.
- Demonstrated commitment to developing multicultural competency and sensitivity.
- Displays positive communication skills when involved in problem solving or disputes with other individuals or in groups
- Skills and competence to establish supportive, trusting relationships with persons with severe and persistent mental illnesses
